Missing/ defaced or broken student identity cards replacement

This is to inform all Students who want to replace their missing Identity Card to present the following items:

1. Police security report on the state of loss of the ID card.

2. Court Affidavit

3. #500 receipt from Uniben Busary.

4. Letter of request to the director Crpu/Ictu, to be submitted at vice chancellor office, Uniben.

5. School fees on-line payment print out

Replacement of dafaced or broken ID cards should present the following.

1. #500 receipt from Uniben Busary.
